Most ALS training teaches you a list of steps.
But in a real resuscitation, the question isn't "what's the next step?" — it's "where is this patient right now?"
There's a simple framework that answers that question every time:
🟡 Pre-Arrest — perfusing rhythm, deteriorating. Your job: prevent the arrest. This is where aspirin, adenosine, cardioversion, and early escalation live. Most patients you'll ever see in ALS practice are here.
🔴 Peri-Arrest — no perfusing rhythm. Your job: high-quality CPR, early defibrillation, reversible causes. Fewer patients, highest acuity.
🟢 Post-Arrest — ROSC achieved. Your job: protect the brain. Titrate oxygen, prevent fever, optimise haemodynamics. ROSC is not the finish line — it's the start of a new set of priorities.
The power of this model isn't complexity. It's the opposite. When you can quickly identify which state your patient is in, you stop second-guessing and start acting with purpose.
