Centenary Occupational Therapy provides an experienced & registered occupational therapist to assess your home safety requirements. We aim to enhance your safety and independence, helping you to keep an active lifestyle and remain in your home for longer. We also provide specialist services relating to chronic diseases.
In home services offered include:
> Falls assessment and prevention education
> Activities of daily living assessment
> Home modification—grab rails, chair raisers, hand held shower heads
> Equipment prescription- toilet aids, shower aids, dressing, eating and transfer aids
> Home safety assessments, including HomeFront assessments
> Energy management specific to chronic disease— Team Care Arrangement
> Pain and fatigue management techniques
GP referral essential with current Medical summary to ensure timely appointment.
